Description:

Plans and conducts segments of treatment programs designed to restore and improve physical social and mental functions while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups.

Requirements:

Day Shift Service area is Chico/Oroville. Day Shift - Mileage is in addition to bill rate. Monday - Friday. Looking for someone with prior home health experience (1 year would suffice). Willing to train if they do no have experience. We service the Butte Yuba and Sutter Counties. Right now the primary need is in Butte County.

Education and Work Experience:

Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016

Licenses/Certifications:

Physical Therapy licensure in all applicable states of practice: Required
Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required

Valid Drivers License (DL) and must be at least 21 yrs of age or older
