Job Summary
Job Summary (List Format):
- Evaluate residents mobility balance strength gait and functional limitations through initial and ongoing assessments.
- Develop and implement individualized person-centered physical therapy treatment plans to improve function reduce pain and prevent disability.
- Deliver direct therapy interventions including therapeutic exercises gait training balance retraining and application of physical modalities.
- Monitor resident progress reassess as needed and adjust treatment plans to achieve therapy goals.
- Participate in interdisciplinary care team meetings and contribute to discharge planning.
- Educate residents families and caregivers on home exercise programs and safety strategies to promote ongoing independence.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides per licensure requirements.
- Accurately document treatment sessions progress notes and billing information in compliance with regulations and facility standards.
- Ensure compliance with state federal and facility documentation and practice standards.
- Support quality assurance and performance improvement initiatives within the rehab team.
- Work in a Skilled Nursing Facility (SNF)/Long-Term Care setting requiring frequent standing bending lifting and hands-on resident mobility assistance.
- Collaborate effectively within a fast-paced team-based rehabilitation environment.
