Job Summary:

We are seeking a licensed Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility setting. The ideal candidate will be responsible for evaluating and treating residents to improve mobility strength balance and overall functional independence.

Key Responsibilities:

Perform comprehensive patient evaluations and develop individualized treatment plans
Provide therapeutic exercises manual therapy and mobility training
Implement fall prevention and strengthening programs
Educate patients and caregivers on home exercise programs and safety techniques
Collaborate with interdisciplinary healthcare teams to coordinate patient care
Maintain accurate and timely clinical documentation

Qualifications:

Active and current state Physical Therapist license
Minimum of 1 year of experience preferred ideally in a Skilled Nursing Facility setting
Strong clinical assessment and treatment planning skills
Excellent communication and teamwork abilities

Additional Information:

Day shift schedule
36 hours guaranteed per week
No holiday coverage required
