Job Summary (List Format):
- Evaluate residents mobility balance strength gait and functional limitations through initial and ongoing assessments.
- Develop and implement individualized person-centered physical therapy treatment plans to restore function alleviate pain and prevent disability.
- Provide direct skilled therapy interventions such as therapeutic exercises gait training and balance retraining.
- Monitor resident progress adjusting care plans as needed to achieve therapy goals.
- Participate in interdisciplinary care team meetings and assist with discharge planning.
- Educate residents families and caregivers on home exercise programs and safety strategies to promote independence.
- Supervise and guide PTAs and therapy aides in accordance with licensure requirements.
- Accurately document treatment sessions progress notes and billing information in a timely manner.
- Ensure compliance with all relevant state federal and facility documentation and practice standards.
- Support quality assurance and performance improvement initiatives as part of the rehabilitation team.
- Work primarily in a Skilled Nursing Facility (SNF)/Long-Term Care setting requiring frequent standing bending and assisting with resident mobility.
- Collaborate within a fast-paced team-based rehabilitation environment.
