We are seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary team in a Skilled Nursing Facility (SNF). The PT will evaluate residents physical conditions create personalized treatment plans and provide rehabilitative care to improve mobility restore function and help patients achieve their highest level of independence.
Key ResponsibilitiesConduct comprehensive assessments of patients physical abilities movement limitations and pain levels.
Develop and implement individualized treatment plans based on evaluation results medical history and patient goals.
Deliver evidence-based interventions to enhance strength balance coordination and mobility.
Collaborate with physicians nursing staff occupational and speech therapists and families to coordinate patient care.
Monitor patient progress update therapy plans as needed and maintain accurate documentation per regulatory guidelines.
Educate patients and caregivers on safe mobility techniques home exercise programs and equipment use.
Ensure compliance with facility policies and state/federal regulations including Medicare/Medicaid documentation requirements.
Participate in care planning meetings and interdisciplinary team conferences.
Supervise physical therapist assistants (PTAs) and aides as needed.
Maintain a safe organized and clean clinical environment.
Graduate of an accredited Physical Therapy program.
Current state license to practice as a Physical Therapist (PT).
CPR certification preferred.
Experience in Skilled Nursing Facilities or geriatric rehabilitation settings preferred.
Strong communication documentation and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Skilled Nursing Facility (SNF) providing care for both long-term and short-term rehabilitation patients.
May involve lifting walking standing and assisting patients with mobility and transfers.
Standard daytime shift: 9:00 AM 5:00 PM
Experience with Point Click Care (PCC) EMR preferred
