Job Summary (List Format):
- Evaluate residents mobility balance strength gait and functional limitations through initial and ongoing assessments
- Develop and implement individualized person-centered treatment plans to restore or improve physical function reduce pain and prevent disability
- Provide direct physical therapy interventions such as therapeutic exercises gait training balance retraining and use of modalities
- Monitor residents progress and modify treatment plans as needed to achieve therapy goals
- Collaborate with an interdisciplinary care team and participate in team meetings and discharge planning
- Educate residents families and caregivers on home exercise programs and safety strategies for independence
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides per licensure requirements
- Maintain accurate and timely documentation of treatment sessions progress notes and billing information
- Ensure compliance with state federal and facility regulations documentation standards and best practices
- Support quality assurance and performance improvement initiatives within the rehab team
- Work primarily in a skilled nursing facility (SNF) or long-term care setting in a fast-paced team-based environment
- Frequently engage in physical activity such as standing bending lifting and assisting with resident mobility
