Position Overview

We are seeking a skilled Physical Therapist (PT) to support rehabilitation services in a healthcare facility in Pleasant Hill California. The Physical Therapist will evaluate patients develop individualized treatment plans and provide therapeutic interventions to improve mobility restore function and enhance overall quality of life.

Key Responsibilities

Conduct comprehensive patient evaluations and functional mobility assessments.
Develop and implement individualized physical therapy treatment plans
Provide therapeutic exercises gait training balance therapy and mobility rehabilitation
Monitor patient progress and modify treatment plans as necessary
Maintain accurate and timely documentation in electronic medical records (EMR)
Collaborate with physicians nurses and interdisciplinary care teams
Educate patients and caregivers on exercises injury prevention and recovery plans
Ensure compliance with facility policies safety protocols and regulatory standards

Minimum Qualifications

Active Physical Therapist license in the State of California
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Current BLS/CPR certification
Strong clinical assessment and patient care skills
Ability to work effectively in a multidisciplinary healthcare environment

Preferred Qualifications

Experience in skilled nursing rehabilitation or post-acute care settings
Experience working with geriatric or orthopedic rehabilitation patients
Familiarity with electronic health record (EHR/EMR) systems
