Position: Stability Focused Case Manager

Reports to: Housing Support Manager

Location: Denver

Type: Full-time

Pay: $25.00 per hour generous benefits including 401k matching 100% employer-paid premiums for health dental vision; paid vacation paid holidays; $40 cell phone stipend

The Stability Focused Case Manager is responsible for supporting individuals in achieving long-term stability through employment education vocational development and benefits navigation. This role provides strengths-based trauma-informed guidance to help community members build income increase self-sufficiency and maintain ongoing housing stability.


Schedule / Hours

40 hours per week with flexibility for occasional evenings and weekends.

Includes time spent in community member-facing work administrative tasks transportation and working collaboratively with internal teams and community partners


Key Area # 1- Individual Case Management

  • Conduct needed program assessments in accordance with program timelines
  • Collaborate with clients to set goals assess needs and provide supportive case management to help them achieve success.
  • Assist with ID and vital document recovery needed for benefits employment and housing.
  • Provide ongoing strengths-based trauma-informed case management using motivational interviewing and problem-solving supports.
  • Connect clients to physical health dental health behavioral health harm reduction substance use services vocational training and employment services available across the All In Mile High system.
  • Make referrals to outside agencies and interdepartmental colleagues such as Peer Support or our on-site Mental Health Professionals.
  • Assist clients in accessing insurance primary dental and specialty care as needed.
  • Transports community members as necessary using personal vehicle for any and all related job description functions.


Key Area #2 - Benefits and Resource Navigation and Income Stability

  • Assist individuals in applying for and maintaining public benefits including Supplemental Nutrition Assistance Program (SNAP) Medicaid Old Age Pension (OAP) Aid to the Needy Disabled (AND) RTD Live SSI/SSDI and other relevant supports.
  • Coordinate with benefits specialists and partner agencies to ensure timely submission and follow-through.
  • Assess strengths interests skills and employment readiness with each community member.
  • Assist with job searches online applications resume development interview preparation and connecting to hiring events.
  • Support clients in accessing vocational educational and workforce development opportunities including GED programs certifications trade schools and higher education
  • Educate and support clients in navigating systems and accessing resources to maintain long-term stability and self-sufficiency.
  • Keep abreast of current trends in community resources social security benefits state government benefits medical and mental health programming and best practices in accessing resources and programs.

Key Area #3 - Data Collection and Documentation

  • Maintain accurate and up-to-date records including HMIS data case notes community member documentation and files and in compliance with data standards.
  • Ensure proper documentation of all activities communications and community member progress.
  • Completes all required forms and reports in a complete and timely manner per program requirements.


Key Area #4 Professionalism and Teamwork

  • Actively participate in case conferencing team meetings training sessions supervisions and community collaborations contributing to a culture of collaboration and accountability.
  • Work collaboratively with internal teams and community partners to support community members stability goals.
  • Actively participates collaborates and contributes to the multidisciplinary team to promote a team culture in which each clients point of view experiences and preferences are recognized understood and respected and in which client self-determination and decision-making in service planning are maximized and supported.
  • Understands and follows the related policies and procedures of the Micro Community operations.


Other Responsibilities

Complete other special duties and tasks as assigned by management.

Physical Requirements

The position requires the ability to perform the essential functions with or without reasonable accommodation. Tasks may involve prolonged periods of standing sitting or walking. Additional

requirements include occasional reaching bending crouching or kneeling along with manual dexterity to operate tools equipment and technology. Visual and auditory ability to respond to tasks and ensure safety are also essential.


Requirements

Qualifications Summary

  • High School Diploma or equivalent and one year either working or volunteering with unhoused individuals in a non-profit setting. Additional education or experience in social work human services or related fields preferred.
  • Experience in case management benefits navigation homelessness services social work or related fields with an understanding of Housing First harm reduction and trauma-informed engagement strongly preferred
  • Ability to work within the community including shelters training sites and office locations.
  • Familiarity with HMIS or similar data systems is a plus.
  • Reliable transportation is required.
  • Cell phone to download and access CVC applications is required

  • Transportation Skills: Comfort with driving and transporting community members and their belongings.
