Avalon Housing is looking for a talented and dedicated person with a commitment to social justice to fill the role of Ashley Intensive Case Manager. The ideal candidate brings both professional expertise and genuine care for the people we serve helping residents move from surviving to thriving.
Using a Housing First approach the Intensive Case Services Team partners closely with residents to promote long-term housing stability and overall well-being. This team collaborates with community providers and internal partners to ensure coordinated person-centered care that addresses mental health physical health social and housing-related needs.
The Ashley Intensive Case Manager (ICM) provides outreach intensive case management care coordination community-building and housing support services within a single-site 24/7 staffed supportive housing setting.
Residents served in this role often experience complex and overlapping challenges including chronic homelessness mental health conditions substance use disorders and chronic medical concerns. The ICM plays a critical role in helping residents navigate these challenges and build sustainable meaningful lives in housing.

Core Responsibilities Include:
- Provide comprehensive case management of the Ashley tenants
- Participate in the interviewing and onboarding of new Ashley team members
- Deliver ongoing proactive and creative strategies to support tenant participation and engagement.
- Conduct comprehensive assessments and collaborate with tenants to develop individual action plans to achieve goals
- Work with tenants and property management to coordinate eviction prevention efforts and develop housing permanency plans.
- Support tenants in developing basic life skills including tenant rights and responsibilities and maintaining a safe and stable apartment
- Assist tenants with accessing needed medical mental health substance use and Psychosocial supports
- Provide mediation and conflict resolution support among tenants and between neighbors
- Provide crisis intervention and de-escalation support as needed
- Understand and uphold all contractual requirements with partner agencies
- Work to build community and peer support among tenants including participation in community programming activities
- Complete all required documentation accurately and in a timely manner including Medication Administration Record (MAR) monitoring and compliance.
- Provide medication management for Ashley House tenants
- Facilitate onsite medication training for Residential Support Specialists
Supplemental Job Duties:
- Participate in on-call rotation and occasional evening and weekend work as necessary
- Assist tenants as needed with activities of daily living such as transportation housekeeping meal preparation medication shopping laundry or other hands-on assistance
Required Qualifications
- Bachelors or Masters degree in social work psychology human services or a related field or a minimum of three years of relevant professional experience
- Training in assertive engagement motivational interviewing harm reduction and trauma-informed care
- One to three years of experience working with similar populations including individuals experiencing homelessness mental illness chronic health conditions and/or substance use disorders

- The ability to show up calm steady and supportive during crises helping residents feel safe and respected
- Strong communication skills both spoken and written with attention to clarity professionalism and compassion
- A positive dependable and team-oriented approach with a strong commitment to helping residents move toward stability
- Strong organizational skills and the ability to balance multiple responsibilities without losing sight of resident needs
- A genuine interest in working with individuals who may be hard to engage using harm reduction and trauma-informed practices
- A demonstrated commitment to equity inclusion and non-judgmental service delivery when working with people from diverse backgrounds

and evening flexibility is required.
