- Engage individuals to form trust-based relationships that foster hope for positive change and personal empowerment
- Offer a positive supportive relationship that helps promote hope, choice, wellness and recovery for participants
- Provide assistance in identifying and linking to, community-based services and supports including outpatient providers, i.e., (primary care, mental health and substance use disorder treatment, housing, financial benefits, etc.) Peer support services will be offered in a mutually agreed upon location in the community or in the consumer's place of residence
- Provide information and support related to the experience of being hospitalized and participating in substance use disorder treatment
- Provide one-to-one and group support during hospitalization. Continue to provide short-term-support services following hospital discharge
