Schedule & Shift
On-Site Coverage:
7 days per week (including weekends and holidays)
Daily rounding required
Shift Structure:
Day Shift: Typically 8 10 hours/day (based on facility needs)
Flexible scheduling between the physician and APRN for coverage
On-Call Requirement:
24/7 on-call rotation (mandatory)
Immediate response availability for urgent/emergent resident needs
Position Overview
This is a full-service staffing opportunity requiring 100% coverage including daily on-site care 7-day per week service and 24/7 on-call availability for a specialized long-term care population.
GSNF is a 100-bed skilled nursing facility providing care to residents with behavioral health developmental and intellectual disabilities requiring a high level of clinical expertise and coordinated care.
Minimum Requirements:
1 Licensed Physician (MD)
1 APRN (or additional MD)
Key Responsibilities:
Provide daily medical rounds for all assigned residents
Deliver routine and emergency medical care
Maintain 24/7 availability for consultation and urgent care
Perform annual physical examinations
Document daily progress notes and clinical records
Collaborate with interdisciplinary teams and facility leadership
Participate in quality improvement initiatives
Ensure compliance with:
HIPAA
CMS regulations
Facility policies and procedures
Qualifications:
Physician (MD):
Active Kentucky Medical License
Board Certified (preferred)
DEA Certificate
Residency completion required
Experience in:
Long-term care
Behavioral health / developmental disabilities (highly preferred)
Current malpractice insurance
APRN:
Active Kentucky APRN License
Prescriptive authority
Collaborative Practice Agreement (if applicable)
Relevant clinical certification
Experience in long-term or behavioral health care preferred
