URGENT Dates of Coverage: April 17-20

DETAILS:

Reason for Coverage: Vacation coverage
Practice Setting: Hospital call coverage
Patient Age Demographic: Adult Geriatric
Procedures Required: General Surgical Cases ADULT

Facility will reimburse hotel ($150/night) and car rental at the standard economy rate.

Board Certified - REQUIRED
ACTIVE OH License & DEA - REQUIRED; no exceptions
BLS/ACLS Certified - REQUIRED
Flu Shot - Required noted at time of name clear
COVID Vaccination - Highly preferred
MUST have a clean background for temps - Required no exceptions
Provider MUST be available for all dates listed; no exceptions. - REQUIRED
