Job Overview:

We arecurrently seeking an experiencedLicensed Practical Nurse (LPN)for an exciting opportunity inMontour Falls NY. This position is based in a long-term care setting and is ideal for a compassionate and dedicated nurse looking to make a meaningful impact on patients lives. The assignment is for13 weeks with astrong potential for extensionbased on performance and facility needs.

Key Responsibilities:

Plan implement and document patient-centered nursing care.
Perform assessments and develop individualized care plans based on patient needs and medical diagnoses.
Prioritize and deliver nursing interventions aligned with best practices.
Provide therapeutic care and monitor patient conditions.
Ensure accurate documentation in accordance with clinical standards and regulatory guidelines.
Collaborate with interdisciplinary teams to deliver holistic care.
Advocate for evidence-based nursing practices and continuous improvement.
Maintain compliance with all health and safety regulations.
Perform other duties as assigned.

Requirements:

Experience:

Minimum2 yearsof recent experience as an LPN in a long-term care or similar clinical setting.

Licenses/Certifications (Required Prior to Start):

ValidLPN license(New York)
BLS Certification(American Heart Association required)
