Director of Case Management

Position Overview
FocusPoint is seeking an experienced Director of Case Management to lead care management and social services operations for one of our healthcare clients. This role is responsible for establishing best practice programs that ensure effective coordination of patient care balancing clinical quality financial stewardship and patient/family needs. The Director will oversee case management and social services teams develop clinical care pathways and foster strong relationships with physicians payers and post-acute providers to ensure seamless transitions of care.
Key Responsibilities
Strategic Leadership
- Develop and implement departmental goals aligned with the clients mission vision and strategic plan.
- Establish best practices in case management and social services to enhance patient experience and hospital throughput.
- Lead utilization review processes monitor length of stay trends and drive resolution of issues.
Financial & Operational Management
- Operate within budget guidelines conduct cost-benefit analyses and identify cost reduction strategies.
- Implement staffing plans cross-training and productivity goals to maximize efficiency.
- Evaluate and recommend technology solutions to enhance case management and denial management functions.
Staff Development & Leadership
- Recruit retain and develop competent staff; provide orientation ongoing training and continuing education.
- Foster a culture of collaboration accountability and professional growth.
- Ensure staff competency assessments and performance evaluations are completed on schedule.
Quality & Risk Management
- Establish and monitor quality improvement plans and performance indicators.
- Participate in denial management committees oversee clinical denials and analyze trends.
- Promote patient safety risk management and adherence to regulatory standards.
Collaboration & Communication
- Build strong relationships with physicians payers and community providers (SNFs home health hospice ambulance services etc.).
- Encourage open communication and teamwork across departments and shifts.
- Represent the department on hospital committees and cross-functional initiatives.
Qualifications
- Licensure/Certification: Current RN license required; specialty certification in Case Management or Administration preferred.
- Education: Bachelors degree in Nursing required; Masters degree in a related field preferred.
- Experience: Minimum of 5 years progressive leadership experience in hospital case management.
- Knowledge & Skills:
- Strong understanding of DRGs Medicare regulations medical necessity criteria and Title 22 requirements.
- Proficiency in budgeting financial management and regulatory compliance.
- Effective communication conflict resolution and staff development skills.
- Familiarity with case management software (e.g. Meditech Interqual) and Microsoft Office.
