Position Summary
- Triage referrals from forensic settings including Community Linkage Hamilton County Justice Center and persons found incompetent to stand trial unrestorable and referred to probate court
- Work with system partners regarding high-risk client care coordination participate in meetings and committees as needed
- Consult with clinicians community partners and staff regarding discharge planning and care coordination for Hamilton County residents specifically high-risk high need dually diagnosed persons
- Manage projects such as Access to Wellness and Community Transition Program
- Complete needs assessment managing utilization of funding and administrative tracking
What We Value in our next Program Coordinator
- Bachelors Degree in counseling social work or other mental health related fields
- Excellent clinical judgement and crisis de-escalation skills
- Clinical assessment skills understanding diagnostic criteria and symptom presentations outlined in the DSM-5
- Strong alignment with trauma-informed person-centered care
- Proficiency with basic office computing tools
- Communicates effectively orally and in writting
- Preferred Ohio license in either social work or counseling
- Preferred working knowledge of the Hamilton County behavioral health system of care
