Position Summary
  • Triage referrals from forensic settings including Community Linkage Hamilton County Justice Center and persons found incompetent to stand trial unrestorable and referred to probate court
  • Work with system partners regarding high-risk client care coordination participate in meetings and committees as needed
  • Consult with clinicians community partners and staff regarding discharge planning and care coordination for Hamilton County residents specifically high-risk high need dually diagnosed persons
  • Manage projects such as Access to Wellness and Community Transition Program
  • Complete needs assessment managing utilization of funding and administrative tracking
What We Value in our next Program Coordinator
  • Bachelors Degree in counseling social work or other mental health related fields
  • Excellent clinical judgement and crisis de-escalation skills
  • Clinical assessment skills understanding diagnostic criteria and symptom presentations outlined in the DSM-5
  • Strong alignment with trauma-informed person-centered care
  • Proficiency with basic office computing tools
  • Communicates effectively orally and in writting
  • Preferred Ohio license in either social work or counseling
  • Preferred working knowledge of the Hamilton County behavioral health system of care
