In-home Assessment Aide (Bilingual English/Armenian)
Job Description:
- Performs and completes In-Home assessment of patients for respite services.
- Assess the safety of the patients home environment.
- Assess the patients functions in activities of daily living.
- Assess other environmental social and behavioral factors that might impact the functions and needs of the patients.
- Gather necessary details and submit information on a timely basis.
- Contact schedule and confirm in-home assessment and respite services.
- Providing superior patient care; comfort safety and confidentiality.
- Communicate effectively with staff and employees for scheduling needs.
- Assist with the coordination of referrals confirmations and billing for respite services.
- Assist with activities of daily living as needed.

License/Certification:
- Driver License
- CPR Certification
- HCA- Home Care Aide (Preferred)
