What Youll Do:
Partner with pediatricians nurses and therapists to support comprehensive patient care
Empower families and caregivers by connecting them with vital services and support networks
Address social determinants of health and remove barriers to care for children in need
Requirements
REQUIRED QUALIFICATIONS
Education: Masters Degree in Social Work
Skills:
Exceptional problem-solving critical thinking organizational interpersonal andwritten/verbal communication skills
Ability to work in self-directed environment with attention to detail follow through and has ahigh level of computer literacy.
History of positive rapport with patients families physicians and interdisciplinary team
License: Active LISW license
PREFERRED QUALIFICATIONS
Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable andexperience that matches the assigned practice specialty is preferred.
