Job Description
A community-based organization is hiring an HCBS Coordinator to support families through the development and implementation of individualized service plans. This role is ideal for someone empathetic organized and eager to help families access and navigate available services.
Responsibilities Include:
Work directly with families to develop and monitor service plans
Coordinate with providers to ensure timely service delivery
Maintain accurate documentation and records
Serve as a supportive point of contact for families
Assist in ensuring compliance with HCBS regulations
Ideal Qualifications:
Strong interpersonal and communication skills
Organized and detail-oriented
Ability to maintain accurate documentation
Compassionate and supportive approach with families
No prior HCBS experience requiredtraining provided
