Responsibilities Include:
Work directly with families to develop and monitor service plans
Coordinate with providers to ensure timely service delivery
Maintain accurate documentation and records
Serve as a supportive point of contact for families
Assist in ensuring compliance with HCBS regulations
Ideal Qualifications:
Strong interpersonal and communication skills
Organized and detail-oriented
Ability to maintain accurate documentation
Compassionate and supportive approach with families
No prior HCBS experience requiredtraining provided
