You will be updated with latest job alerts via emailThe IBHH Care Coordinator will be a part of the Integrated Health Home (IBHH) Team which is accountable for an individuals total care and will deploy a multidisciplinary team-based approach to address the physical behavioral and social health needs for attributed individuals utilizing a single Health Action Plan (HAP). Services delivered by the IBHH Team are highly individualized address complex and chronic conditions and capitalize on available health information and actionable data through the use of technology and appropriate information sharing methods. Services may be delivered via a mix of face-to-face and telephonic or electronic interventions but must ultimately be tailored in a manner that is unique to the individuals needs so as to maximize engagement and activation.

Must be 21 years of age (automobile insurance requirement) and possess a valid AZ drivers license with a clean or acceptable driving record.
Applicants must be able to pass a pre-employment drug screen and background check.
