Job Description

The Pediatric Care Coordinator and (PCC) promotes improved access and are management for clinic pediatric patients by acting as the liaison between the client and the community and will help dissolve the silos between the Medical and Behavioral Health (BH) departments within the health centers. This position is an essential member of the primary care team facilitating streamlined integration of medical behavioral and social care for pediatric patients and coordinating services and follow up with patients and providers. The PCC will also assess patient/family needs connecting them to preventative care resources/programs advocating for patients and otherwise help patients navigate getting the holistic allaround care they need to live healthy and balanced lives from AHS and other agencies.

Essential Job Functions

    • Coordinate BH and developmental needs for pediatric clients with a focus on preventative and comprehensive care
    • Support the navigation of wraparound prevention serves for clients and families including enrollment in MediCal CalFresh housing assistance eviction prevention and legal aid
    • Assist with implementing universal BH ASQ/MCHAR and trauma screenings for pediatric populations with the goal of early intervention for atrisk clients
    • Link client to appropriate followup resources both within the health centers and throughout Alameda County
    • Encourage partnership between external entities such as schools ACHH contracted services HelpMeGrow California Childrens Services (CCS) and the Regional Center of the East Bay
    • Advocate for AHS patients and their families to assure the availability and utilization of appropriate community services
    • Actively participate in ongoing training and workshops that promote continuing professional growth and increase knowledgebased around pediatric healthcare services
    • Communicate well with clients the document clearly in the medical record and interface with medical providers to advocate for the client and their family
    • Support a positive work environment and foster teamwork
    • Responsible for collecting and entering data regarding services provided within the clinic and outside of the clinic via referral programs

General Agency/AHS Duties

    • Fosters an environment that promotes trust and cooperation among clients and staff.
    • Enforces clinic policies and procedures including maintenance of client confidentiality under HIPAA privacy rules to ensure that the principles of AHS are implemented.
    • Plays an active role in planning organizationwide activities such as AHS general membership meetings fundraisers special clinic days and other events.
    • Participates in general membership meetings fundraisers and other public events as required.
    • Participates in outreach activities agency advocacy and serves on ad hoc committees as requested.
    • Attends AHS staff retreats and Board of Directors meetings as required.
    • Participates in agencywide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency.

Minimum Qualifications

    • High school diploma/AA and at least two years of progressively responsible work experience in a communitybased setting
    • Bilingual/bicultural in an Asian language required Cantonese and/or Mandarin.

Preffered Qualifications

    • BA preferred
    • Bilingual in an Asian language is highly preferred.

Knowledge Skills & Abilities

    • Competent leadership and administrative skills.
    • Flexibility and creativity this is a pilot program and will be an iterative and evolving process.
    • Excellent communication organizational and time management skills with the ability to work individually and as a team.
    • Proficiency in computer operations navigational skills and comfortable with Internetbased applications.
    • Sound decisionmaking skills including problemsolving critical thinking and good clinical judgment for clinical and nonclinical issues.
    • Ability to work effectively and collaboratively with a variety of customers including clients family members health center/office staff providers and communitybased organizations
    • Cultural humility and respectful curiosity ability to meet clients and families where they are
