MSW/LICSW
Experience in the field ofGeripsych substance abuse and/or community mental health services preferred.
Clinical experienceunderstanding of and comfort working with patients of all ages who suffercomplex medical and psychiatric problems; ability to work with the families ofsuch patients and ability to help patients and families understand and accessthe resources required to support care.
Strong understanding ofpsychiatric and family system problems and ability to use this understandingto formulate succinct case summaries.
Strong assessment andtreatment skills

GENERAL SUMMARY/ OVERVIEW STATEMENT:
The clinical social worker isa key member of the Outpatient Behavior Neurology team who provides andoversees the provision of psychosocialcare for selected patients and families. Conducts biopsychosocial assessments providesintervention and treatment as indicated. Identifies highrisk psychosocial factors ofpatients/families that impact health status and helps hospital staff understandthe influence of those factors upon the course of medical care. Assumesclinical evaluation intervention and planning for patients with complexpsychosocial risk (homelessness protective services frail elderly disabledpsychiatric and substance abuse etc.. Works effectively as partof interdisciplinary health careteams. Provides clinical services topatients/families that address psychosocial environmental age specific andcultural issues in order to maximize emotional social and physical well beingand the effective use of health care and community resources. Collaborates with and provides social workconsultation to other disciplines within the setting and community. Participateson Departmental Hospital Satellite community task forces and committees.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Clinical Practice:
Provides assessment of patients to evaluatemental health/psychiatric history/emotional issues/coping style understandingof illness/adjustment/compliance barriers to care cultural issuesabuse/neglect and domestic violence.
Provides psychosocial assessment of familiesto determine: family relationships/systems as they relate to care of thepatient. Identifies family decision makers and caregivers; family understandingof illness and trajectory of care. Identifies family coping style familyresources and cultural issues.
Employs a range of clinical interventionssuch as individual group or family counseling.Provides caregiver/family counseling/support to promote familycohesiveness to provide care to patient and prepare families for end of life.Advocates on behalf of patients and families to gain access to services andresources. Refers patients to otherproviders as necessary.
Develops comprehensive biopsychosocialassessments responsive to age appropriate and cultural needs and concerns. Employsa range of clinical interventions such as psychotherapy (individual couplesfamilies and group) psychosocial counseling crisis intervention carecoordination complementary therapies information and referral and safetyplanning. Advocates on behalf ofpatients and families to gain access to services and resources.
Provides mandated assessments when abuse issuspected (child disabled adult elder) and safety assessment when domesticviolence is reported. Files reports asindicated.
Identifies patients psychosocial financiallegal psychiatric or substance use that effect patient care management and collaborateswith the team to facilitate patient care process.
Works effectively as part of theinterdisciplinary health care team communicating regularly with the team andother members on cases and as issues arise. Documents timely and relevantinformation.
Coordinates family/team meetings as neededand when appropriate. Provides psychosocial consultation on patient careplanning and patient/family management and community resources. Implementspsychosocial programs based on patient/family identified needs.
Facilitates the appropriate and efficient useof hospital and community resources.
Participates in formal and informal clinicalcase reviews clinical supervision educational seminars and research projects.
Teachingand Education:
Assesses patient/family learning needsstyles and readiness. Educatespatients/families based on treatment plan identifies barriers to carediversity issues and learning styles.
Mentors and may supervise students andstaff. May teach in Departmental andHospital seminars workshops and rounds.
Collaboration:
Collaborates and consults with patientfamilies and providers within the Practice Setting and community to develop andimplement patient care plans. Refers toand collaborates with Departmental programs outside agencies facilitiespayors and other relevant community resources.
Organizational/AdministrativeSkills:
Takes responsibility for own administrativeduties including timely and appropriate documentation in patient medicalrecords timely and accurate daily reporting of activities and Hospitalsscheduling systems and accurate reporting of time worked.
Attends and participates in Staff Meetingsand interdisciplinary meetings/rounds.
Participates in Departmental and Hospital committees.
