Job Description

3 Month Contract with a local Authority

Job Purpose

  • The role involves promoting and maintaining independence for vulnerable adults while safeguarding them from abuse through established protective procedures. It includes assessing the needs of individuals and their carers in accordance with the Care Act (2014) collaboratively developing personal budgets and support plans and ensuring that services and direct payments are regularly monitored reviewed and evaluated to meet identified needs effectively.


Key Responsibilities

  • Ensuring the ethos of wellbeing is adhered to when working with vulnerable adults and their carers.
  • Participate in the operation of duty and respond accordingly to need which will involve Safeguarding enquiries and Best Interest Assessment (BIA) when required.
  • Provide an assessment service to individuals their carers and other agencies in accordance with Care Act (2014) criteria.
  • Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers in conjunction with professional colleagues and representatives of voluntary and private agencies.
  • Supervise and support staff as agreed with Line Manager and in line with Council procedures.
  • Offer support and advice as appropriate to service users and carers.
  • Through the assessment and prioritisation of need commission appropriate services and authorise expenditure on services within designated limits.
  • Monitor the efficiency and effectiveness of individual care and support plans.
  • Review the needs of individual service users and carers.
  • Maintain client records in accordance with Council policy.
  • Respond to and where possible resolve complaints within the Council s complaints procedure.
  • Respond to new and changed legislation in respect of Adult Services and implement as directed.


Essential experience Required

  • Relevant experience of working within community care with vulnerable adults in the community and/or hospital settings.
  • Working across a range of agencies and understanding their role.

Essential Qualifications Required

  • Social Work Degree/CQSW/CSS/DipSW/OT/RGN/RNMH/RMN with community Experience.
  • For qualified Social Workers registration with the Health and Care Professionals
  • Council is mandatory from 1 December 2012.
  • Those holding a social work qualification from 2012 onwards must provide evidence of successfully completing an assessed and supported year in employment (ASYE) OR if newly qualified undertake to an assessed and supported year in employment as part of the council s probationary process.
  • Either be qualified to undertake best interest decisions or be willing to undertake

  • This Role is 36 Working Hours Per Week
  • This Role Requires An Enhanced DBS
