Our client is seeking a fulltime RN Case Manager for Home Health in East Cuyahoga County OH. The ideal candidate will provide skilled nursing care demonstrate effective communication and collaboration and maintain patient safety. This role requires strong organizational skills the ability to work independently and a commitment to maintaining confidentiality. The candidate should have experience in home health care. This position will service the following coverage areas; Bratenal East Cleveland Cleveland Heights University Circle and surrounding areas. Key Responsibilities Provide direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups Utilize principles of the nursing process to assist in data collection planning and evaluation of nursing care Demonstrate nursing knowledge and skills in the implementation of prevention therapeutic and rehabilitative nursing care Utilize effective interviewing techniques; sets limits and collects data based on specific role Communicate and collaborate effectively with health care providers third parties patients and families based on specific role Maintain awareness of comfort and safety needs of patients Observe patients records significant conditions and reactions and notify supervisor or Physician of patients condition and reaction to drugs treatments and significant incidents Respond to lifesaving situations based upon nursing standards policies procedures and protocol Use oral and written communications to convey pertinent information to members of the healthcare team in a timely manner EMR: Home Care Home Base Equipment Provided: Tablet Phone Schedule and Shift Details 8:30a4:30p 1 weekend per month Including working one summer and one winter holiday
Requirements
- Associate degree or higher
- Home Health and/or Med Surg experience req
- 1 years of experience preferred in ANY of the following:
- Acute Care
- Emergency Department (ED)
- Critical Care
- Burn Unit/Wound Clinic
- Oncology
- Acute Care
- Valid Registered Nurse (RN) certification/license in the state of Ohio (OH)
- Valid American Heart Association Basic Life Support (AHA BLS) certification/license
