Summary:
We are seeking multiple Registered Nurses (RNs) in the Greater Boston Area who are interested in permanent Case Management roles to start ASAP. These are NOT remote positions. Must be onsite. New graduate RNs are welcomed to apply, no experience required! We are open to part-time candidates as well (minimum 25 hours per week).
Responsibilities:
- Review of care plans, and participating in assessments
- Developing assessments frequently
- Discharge planning
- Updating service plans
- Collaborate with interdisciplinary team
- Supervising CNAs
Requirements:
- Associates in Nursing, or BSN
- MA RN licensed
