Job Title: Case Manager - Homeless Services

Pay 24.50 - 27.50 per hour

Location: New York City, NY

Department: [Social Services / Homeless Outreach / Community Support]

Reports To: [Director of Homeless Services, Senior Case Manager]

Position Type: [Full-time/Part-time]

Job Summary:

Carter Lard Consulting is seeking a compassionate and dedicated Case Manager to join our Homeless Services team. The ideal candidate will have at least 2 years of experience working directly with homeless populations, with a strong understanding of the challenges faced by this community. Bilingual abilities are highly preferred, as they will facilitate better communication with a diverse client base.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs, including health, emotional, social, and environmental factors.
  • Develop and implement personalized case plans that address clients' specific circumstances, with the goal of achieving stable housing and self-sufficiency.
  • Provide ongoing support, counseling, and advocacy to clients, assisting them with accessing community resources and services such as housing, employment, healthcare, and education.
  • Maintain regular contact with clients through office visits, home visits, and outreach programs.
  • Collaborate with other service providers and stakeholders to ensure a coordinated approach to client care.
  • Monitor and document client progress, maintaining detailed records and preparing reports as required.
  • Conduct crisis intervention as necessary and connect clients with appropriate emergency services.
  • Participate in community outreach and education efforts to raise awareness of homeless issues and services.
  • Conduct bilingual services when necessary, providing translation and interpretation support to facilitate communication with non-English speaking clients.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Master's degree preferred.
  • Minimum of 2 years of experience working with homeless populations.
  • Bilingual skills are strongly preferred (please specify language, e.g., Spanish, Mandarin, etc.).
  • Knowledge of issues pertaining to homelessness, including mental health, substance abuse, domestic violence, and poverty.
  • Experience in case management software and proficiency in Microsoft Office Suite.

Skills:

  • Strong interpersonal and communication skills, with the ability to engage with diverse populations.
  • Effective organizational and time management skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Crisis management skills and the ability to handle difficult situations with sensitivity and professionalism.
  • Knowledge of community resources and services for homeless individuals.

Working Conditions:

  • Work is generally performed within an office environment but includes potential for exposure to adverse conditions while engaging in outreach activities.
  • Flexibility to work outside of traditional office hours as needed to meet clients' needs.
