The pay for this position is $44850-$58908.9150 annually.
$500.00 sign on bonus

The Community Case Manager works directly with individuals receiving services connecting them with resources documenting client progress and supporting follow-through on treatment plans and referrals

- An Associates Degree and two years experience and/or Bachelors Degree.
- The ability to establish and maintain supportive relationships with children youth and families.
- Knowledge on how to provide holistic and therapeutic case management services.
- Experience working in the mental health field.
