About the Role
The Supports Coordinator Assistant - Adults helps individuals access services resources and supports that promote independence and community inclusion. This role works under the supervision of a Qualified Supports Coordinator or Manager and adheres to Person-Centered Planning principles and regulatory requirements.
Key Responsibilities
- Assist with implementing Individual Plans of Service (IPOS)
- Link coordinate and monitor services and supports
- Help individuals maintain entitlements (Medicaid SSI etc.)
- Support crisis planning and resolution
- Coordinate health and safety strategies including discharge planning
- Promote community inclusion self-advocacy and relationship building
- Assist with housing transportation and employment resources
- Complete timely documentation and maintain compliance with agency and funder requirements
Qualifications
- Education:
- Bachelors degree in Human Services (eligible for advancement to Supports Coordinator after one year)
- OR Bachelors degree in non-human services field
- OR Associate degree or 60 credit hours plus 2 years of experience with individuals with I/DD
- Strong communication organizational and computer skills
- Valid Michigan drivers license insurance and acceptable driving record
Additional Requirements
- Extensive travel within Wayne County; reliable transportation required
- Ability to work flexible hours and maintain documentation standards
- Completion of job-specific training provided by CLS
