Youll be part of the therapy specific team supporting Home and HeartHome Health Care Agency working within a collaborative multidisciplinary model. Care is coordinated patientcentered and teamdriven with shared accountability and open communication.

Job Overview & Work Site
Youll work with a diverse caseload including a higherthanaverage postoperative orthopedic population.

License & Experience
Requirements for this position include the following:
- Graduate (current or pending) from an accredited physical therapist assistant program
- Active or pending state physical therapist assistant license
