Physical Therapist (Permanent Placement)

Location: Piscataway NJ

Type: Full-Time Permanent

Pay: $40.00/hr $50.00/hr

Shift & Schedule: Monday Friday 8:00 AM 4:30 PM

Position Overview

We are seeking a compassionate and dedicated Physical Therapist to join a rehabilitation team in a skilled nursing and post-acute care setting. This role is responsible for evaluating patients developing individualized treatment plans and delivering evidence-based therapy interventions designed to improve mobility strength balance and functional independence.

Responsibilities

Evaluate residents and develop individualized treatment plans.

Implement evidence-based physical therapy interventions.

Improve patient mobility strength balance endurance and functional independence.

Monitor patient progress and adjust treatment plans as needed.

Collaborate with physicians nurses occupational therapists speech therapists and interdisciplinary team members.

Maintain accurate and timely clinical documentation.

Educate patients families and caregivers on treatment plans and home exercise programs.

Ensure compliance with facility state federal and professional practice standards.

Participate in care planning meetings and interdisciplinary team discussions.

Promote patient safety and quality outcomes throughout the rehabilitation process.

Required Qualifications

Graduate of an accredited Physical Therapy program.

Current New Jersey Physical Therapist License or eligibility for licensure.

Strong clinical evaluation and treatment skills.

Excellent communication and patient care abilities.

Preferred Qualifications

Experience in Skilled Nursing Facilities (SNF).

Experience in rehabilitation or post-acute care settings.

Experience working with geriatric and medically complex patients.

Knowledge of rehabilitation documentation systems.
