1 year contract with local authority:
This position is for a Mental Health Social Worker within the local authoritys mental health social care team focusing on facilitating hospital discharges from mental health facilities. The role is crucial in managing complex adult mental health cases and requires expertise in statutory Care Act tasks including assessments reviews and care planning. The Social Worker will collaborate with multidisciplinary professionals and engage in various legal and safeguarding activities to ensure comprehensive care and support for individuals.
Responsibilities:
- Manage complex adult mental health cases.
- Conduct statutory Care Act assessments reviews and care planning.
- Collaborate with multidisciplinary teams including attending ward rounds.
- Work with Housing and brokerage services.
- Handle Court of Protection applications and Mental Health Tribunals.
- Conduct Safeguarding Enquiries and participate in duty work.
- Facilitate hospital discharges from mental health facilities.
Requirements
Requirements:
- Proven experience in managing complex adult mental health cases.
- Experience working in mental health within a Local Authority.
- Proficiency in completing statutory Care Act tasks.
- Ability to work alongside multidisciplinary professionals.
- Experience with S117 Court of Protection applications and Mental Health Tribunals.
- Capability to conduct Safeguarding Enquiries.
- Must be a driver with access to own transport for travel within and outside the borough.
- Ability to work under pressure manage complex cases and prioritize workloads effectively.
- Note: This role is not a Care Coordination position.
