Job Summary

Liquid Personnel is recruiting anAdult Social Worker to join its clients Integrated Network team based in Bridgend.

This is an exciting time to join our Integrated Cluster Network Teams. The Cluster Team work and support to develop a team around the person and their family or those important to them. The people we support can have long term complex challenges around their physical health mental health and emotionalwell-being. It works with both older and younger adults. We aim to provide a holistic support to allow to live their lives as they wish to and ensure voice choice independence and control in their lives.

What will your responsibilities be

Undertakestrengthsbased personcentred assessmentsunder theSocial Services and Wellbeing (Wales) Act 2014 identifying eligible care and support needs for adults and carers
Develop implement and reviewcare and support plansthat promote independence wellbeing and community inclusion
Complete assessments relating to hospital admission discharge planning community living and mental health needs
Support individuals to access appropriate services including domiciliary care reablement residential care equipment adaptations and direct payments

Qualifications and Experience:

To be successful in this role you must have.

Professional Qualification:MA BA BSc in Social Work Diploma in Social Work or CQSW.
Registration:Must be registered withSocial Care Wales.
Experience: 2 years post-qualified experience working in a statutory setting.
Legislation Knowledge:Strong understanding of theMental Health Act 1983Mental Capacity Act 2005Social Services and Wellbeing (Wales) Act 2014 safeguarding procedures and the Mental Health (Wales) Measure 2010.
Skills:Excellent assessment communication and riskmanagement skills with the ability to work effectively in a fastpaced multidisciplinary environment.
Driving Licence: Full UK driving licence and access to a vehicle is often required for community visits.
