Adult Social Worker Mental Health
Job Summary
Liquid Personnel is currently recruiting for an experienced Social Worker to join its clients Integrated Cluster Network Team based in Bridgend Wales.
This is an exciting opportunity to work within a multidisciplinary service that supports adults with longterm complex needs relating to physical health mental health and emotional wellbeing. The team works with both younger and older adults adopting a strengthsbased personcentred approach to ensure individuals are supported to live their lives with choice independence and control.
Responsibilities
- Undertakestrengthsbased personcentred assessmentsunder theSocial Services and Wellbeing (Wales) Act 2014 identifying eligible care and support needs for adults and carers
- Develop implement and reviewcare and support plansthat promote independence wellbeing and community inclusion
- Complete assessments relating to hospital admission discharge planning community living and mental health needs
- Support individuals to access appropriate services including domiciliary care reablement residential care equipment adaptations and direct payments
- Work with adults experiencingmental health difficulties contributing to assessment care planning and recoveryfocused intervention
- Complete or contribute toCare and Treatment Plansunder the Mental Health (Wales) Measure 2010
- Work in partnership with health professionals within multidisciplinary teams
- Where qualified undertake duties as anApproved Mental Health Professional (AMHP)and contribute to Mental Health Act assessments
Requirements
- Professional Qualification:MA BA BSc in Social Work Diploma in Social Work or CQSW.
- Registration:Must be registered withSocial Care Wales.
- Experience: 2 years post-qualified experience working in a statutory setting.
- Legislation Knowledge:Strong understanding of theMental Health Act 1983Mental Capacity Act 2005Social Services and Wellbeing (Wales) Act 2014 safeguarding procedures and the Mental Health (Wales) Measure 2010.
- Skills:Excellent assessment communication and riskmanagement skills with the ability to work effectively in a fastpaced multidisciplinary environment.
- Driving Licence:Full UK driving licence and access to a vehicle is often required for community visits.
