The Health Coordinator is responsible for a team of Health Advocates working to ensure that all children in their assigned centers are up to date on EPSDT requirements receive health follow-ups on acute and chronic conditions and have medical and dental homes.

What Youll Do:

- Provides coaching and support to family services and health staff to assure the reliability of family assessment and goal progress data and to strengthen staff focus on outcomes-focused family interventions.

- Oversees Health and Disabilities Advocates to assure that all children in the program are up to date on Early Periodic Screening Diagnostic and Treatment (EPSDT) preventative health care requirements and receive timely follow up evaluation and treatment.

- Oversees Health and Disabilities Advocates to assure that all families have quality medical and dental homes and maintain consistent health insurance coverage if eligible.

- Provides guidance and support to Health and Disabilities Advocates to complete Individual Care Plans for children with chronic health conditions and follows up to ensure that childrens health conditions are effectively managed.

- Supports family services and health staff to assist families to set individualized evidence-based goals and make demonstrable changes in family life practices linked in the research to school readiness.

- Works in coordination with program leadership to develop and implement targeted recruitment strategies that assure enrollment of children with greatest need including children of working families eligible for childcare subsidies.

- Coordinates with Center leadership to assure that family services health and early learning teams work together to assure on-time daily attendance of all children and to implement strategies to reduce chronic absence.

- Utilizes qualitative audit and external evaluation data to track child and family outcomes and to refine strategies to enhance family impact.

- Works with Director of Family Services and Health and other program leadership to establish monitoring systems to oversee EPSDT adherence including following up on expired or coming due preventive health visits and follow up.

- Develops and aligns program wide health and wellness campaigns with agency mission; including promoting consistent family routines that support healthy lifestyles and habits.

- Works with program-wide leadership to build a staff and agency health and wellness culture to model and support our health goals for children and families.

- Supervises assigned department personnel. Includes the following responsibilities but are not limited to: hiring training scheduling evaluating and ensuring accuracy timeliness and the completion of all work performed by direct reports.

- Educates direct reports on all department and company policies and procedures.

- Meets with assigned staff regularly to identify and resolve problems manage projects track goals and review work processes and procedures.

What Youll Bring:
1. Bachelors degree in Public Health Health Education and Promotion Family and Child Development or related field required; Masters Degree or R.N. preferred.

2. At least 3 years of experience working with diverse families in low-income communities on health and wellness interventions including evidence-based strategies to reduce health disparities with supervisory responsibilities.

3. Background and understanding of child preventative health including EPSDT requirements for children 0-5 years old.

4. CPR and First Aid Certifications or ability to obtain within 1 year of employment.

5. Physical exam and background checks are required for this position.

6. Travel required locally or long-distance up to 10% of the time for work-related meetings and functions.

7. Must have a valid drivers license and/or reliable transportation.
