Alfred Health is a leader in health care delivery improvement research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne from ambulatory to inpatient and home and community-based services. On 1 January 2026 Alfred Health will come together with Kooweerup Regional Health Service Peninsula Health Bass Coast Health and Gippsland Southern Health Service to create Bayside Health a newly merged health service with more than 22000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce providing new training education and career opportunities. Subject to final approval on 1 January 2026 all employees from the five health services will transfer to Bayside Health.

The Department

The Caulfield Community Health Service (CCHS) provides a range of clinical and health promotion services for people of all ages who live work or study within the local community. These multi-disciplinary services are provided in the centre community home or via operates from the Caulfield Hospital site.

The Role

The job will involve comprehensive assessments and time limited interventions. The majority of the clients will be over 65 and referred via My Aged Care under the Commonwealth Home Support Program.

The Grade 1 Physiotherapist position will sit within the Adult Health team and you will:

  • Manage a diverse caseload including musculoskeletal neurological pain and chronic conditions
  • Develop and implement person centred care plans
  • Work within a multi-disciplinary team to ensure a holistic approach to client support
  • Liaise with relevant agencies to coordinate facilitate and communicate client treatment and outcomes
  • Participate in regular supervision with the discipline senior to ensure quality professional practice

Qualifications and Experience Required

Essential

  • Tertiary qualification in Physiotherapy
  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA) and adherence to continuing professional development requirements
  • At least 1-2 years post qualifying clinical experience as a Physiotherapist
  • Current Victorian Drivers Licence (unrestricted)
  • Capacity to work autonomously and yet facilitate a collaborative culture
  • Excellent communication skills (written and oral) and problem-solving skills

Desirable

  • Credentialed with SWEP/Geat2go for equipment / modification prescription.
