The Program Coordinator for Population Health is responsible for population health programs that promote the delivery of patient centered care. This includes educating stakeholders on program applications collecting information and submitting required documents while protecting PHI. Analyzes program needs and system care delivery practices to determine workflows tools and resources that are needed to be successful. Assists and trains practice teams on program deliverables timelines and reporting. Develops and maintains training materials. Analyzes and reviews clinic performance with leaders and care team members monthly. Coordinates workflow design by facilitating meetings with clinic teams content experts leaders and IT to understand workflow gaps and improvement needs. Responsible for tracking and completing workflow changes and participates in communication and training of such changes.
Job Requirements
In addition to bringing your whole self to the workplace each day qualified candidates will need the following:
- 3-5 years of health care experience.
- Experience with process improvement methodologies.
- Experience with an electronic health record.
Team-building/facilitation of meetings.
- Experience with change management.
- Proficient use of Microsoft Office Suite (Word Excel PowerPoint Outlook SharePoint).
- Development and implementation of training materials and presentations.
- Lean or Six Sigma experience (preferred)
- Baccalaureate degree in nursing health sciences or business required
- Masters preferred
- Project management certification (preferred)
Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)
