Job Summary

RATES:

Malpractice Must be included in rates.

Orientation $107.50 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $215 and below - Per hour not OT

Holiday $322.50 - Must be 1.5x the hourly rate. Per holiday hour no OT

Schedule/Availability Requirements:

Schedule: Monday - Thursday FT for 12 months

Shift times: 7:30 am - 5:30 pm

No call

Duties: All aspects of primary care and management of established patient panel. Chronic care management.
