General Summary:
This is a direct service position requiring knowledge of mental health community programming and entitlement programs. Primary responsibilities include the provision and/or coordination of assessing and monitoring client needs developing service plans linking clients to services and resources in the community and advocating for clients as necessary.
Principle Responsibilities and Duties:
- Assist clients through program enrollment and orientation processes including required paperwork.
- Provide and/or coordinate the assessment of client needs development of individual service plans (as assigned) linkages to internal and external services and resources and natural support systems and advocacy.
- Facilitate effective communication among various program components and with external service providers and natural support systems.
- Address client concerns as they arise i.e. resource coordination appointments transportation housing entitlements coordination debt money management etc.
- Transport and accompany individuals to appointments as needed.
- Observe and report symptoms of decompensation to appropriate staff and agencies.
- Plan and facilitate discharges.

What we need from you.
- One of the following combinations of education and experience:
- A high school diploma or GED and 2 years of related experience.
- A bachelors degree and 1 year of related experience.
- The willingness to obtain additional certifications and trainings as required.
- A drivers license with 3-points or less and access to an insured vehicle.
- Must have availability to work days nights weekends and holidays depending upon need of the program.
