Job Description
Are you passionate about helping patients regain independence and return home safely after a hospital stay A renowned healthcare system in New Hampshire is seeking a compassionate and skilled Post-Acute Care Registered Nurse (RN) to join its growing team.
This brand-new transitional care unit provides short-term recovery and rehabilitation services for patients following an acute hospital admission. The interdisciplinary team offers physical occupational speech respiratory and recreational therapy all working together to support patient recovery and a smooth transition home.
Key Responsibilities
- Provide comprehensive nursing care using the nursing process including assessment planning implementation and evaluation.
- Apply clinical judgment and critical thinking to guide care decisions and interventions.
- Conduct detailed patient assessments and use data to develop individualized care plans.
- Document patient progress accurately and adjust plans as needed to meet goals.
- Educate patients and families on recovery plans self-care and post-discharge instructions.
Qualifications
- Graduate of an accredited nursing program
- Active RN license
- Strong communication teamwork and patient education skills
Position Details
- Full-time permanent position
- Schedule: 3x12-hour shifts (nights or day/night rotation available)
