Job Summary

Home Health Registered Nurse (RN)

We are seeking an experienced Home Health RN to provide skilled nursing care to adult and geriatric patients in a home health setting. The ideal candidate will have 1 year of skilled Home Health experience with OASIS proficiency required and the ability to work independently while delivering high-quality patient-centered care.

Key Qualifications & Requirements:

Specialty: Home Health
Experience: Minimum 1 year of skilled Home Health experience (OASIS required)
Certification: Active BLS required
Charting System: Epic experience preferred
Scrubs: Any color permitted

Role Expectations:

Perform comprehensive assessments including Start of Care routine visits recertifications and discharges
Provide medication administration and management wound care and patient education
Accurately document care using Epic and comply with all clinical and regulatory standards
Collaborate with interdisciplinary teams to support optimal patient outcomes
