Job Summary
POSITION SUMMARY: The Hospice Nurse Case Manager admits patients to service assesses the patient s and family s physical psychosocial environmental safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patients environment. This role coordinates care with a multidisciplinary team participates in the hospice programs quality assessment performance improvement program and conducts follow up visits as needed. 1 Year of Hospice Nursing Experience Required Territory Details: Atherton Menlo Park Palo Alto Mountain View Sunnyvale Santa Clara San Jose Saratoga Los Gatos Campbell Morgan Hill Gilroy and neighboring cities as needed.
Shift: Days: 8:30am-5pm (Schedule can be flexible based on patients availability.) Weekends: One weekend per month.
