Housing Coordinator FACT Team

As a member of the BH Forensic Assertive Community Treatment (FACT) Team this position has the primary duty is to work closely with and assist individuals increase their self- sufficiency and maintain their housing arrangements. Individuals can develop their self-efficacy based on a home visitation model utilizing relational theory and stages of change concepts ultimately equipping clients with skills to sustain sobriety from addition and trauma.

Required Education: Bachelors Degree
Defined Education: Health or Human Services as applicable for QMHP-CS credentialing
Required Years Experience: Two (2) years experience
Defined Experience: Two years experience in Social Services field
License/Certifications: Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.
Valid TX drivers license with an acceptable driving record
