Responsible for implementing the clinical component of community health activities provided by the Health and Wellness Services Department. Works closely with all members of the Health and Wellness team. Responsible for assuring that JCAHO standards and regulations are met in clinical functions conducted by Health and Wellness accordance with the mission and philosophy of Dignity Health the Community Health Nurse is a professional caregiver who assumes responsibility and accountability for the assessment planning implementation and evaluation of care for patients and their families. This position may have access to third party credit card information and transactional systems (cash registers point of sales devices applications supporting credit card transactions and reports or other documents containing credit card information) from multiple transactions or reports and/or files containing bulk transactional information containing unencrypted or un-redacted credit card information.

Requirements:

• Minimum of 3 years experience in clinical setting
• Completion of annual safety training
• Completes and maintains required unit specific training
• 70% Travel Required

Licensure:

• Valid license to practice as a registered nurse in the State of California
• BLS Certification from American Heart Association

Preferred:

• Experience in community health nursing preferred
