Job Summary:
The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients providers and other members of our team.

Required Skills & Certifications:
- Active Registered Nurse license in the District of Columbia (DC) or applied (2 weeks prior to being submitted to job)
- Basic Life Support (BLS) certification
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum of 2 years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
Preferred Skills & Certifications:
- CPI (Crisis Prevention Intervention) certification
- Child-friendly demeanor
Special Considerations:
- Pediatric Behavioral Health environment
Scheduling:
- 7:00 AM to 7:00 PM or 7:00 PM to 7:00 AM shifts
- Every other weekend and every other holiday rotation
- Average daily census: 20
- Rooms: 23
- Ratio Days: 1:5
- Nights: 1:3
- Weekends 1:5
