The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness within the Emergency Room. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. The role requires strong communication and collaboration with patients providers and other team members.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills

Special Considerations:
- Variable shift start times can be 10 AM 12 PM or 7 PM primarily evenings to nights.
- Weekends are required on a rotating basis.
- On-call is not required.
- Floating between units is not required.
