Overview: Provide palliative care services as a Nurse Practitioner supporting patients in facilities and community settings in and around Mechanicsburg PA. This role focuses on delivering comprehensive patient care plans collaborating with interdisciplinary teams and supporting patients and families through compassionate high-quality palliative care in a supportive and growth-oriented environment.
Responsibilities:

Manage and deliver patient care plans in facilities and community settings

Collaborate with interdisciplinary care teams to ensure coordinated care

Support patients and families in Mechanicsburg PA and surrounding areas
-
Perform clinical assessments and ongoing evaluations
-
Provide symptom management and patient-centered palliative care
-
Maintain accurate and timely clinical documentation

Qualifications:

Masters degree or higher in nursing
-
Active Registered Nurse and Nurse Practitioner licenses in Pennsylvania

Minimum of one year of experience in hospice palliative care or home health

Minimum of five years of overall clinical experience

AHA BLS or ACLS certification
-
Experience in community-based home health or hospice/palliative care strongly preferred
