Overview: Provide in-home healthcare services as a Licensed Practical Nurse supporting patients in the Milford PA area. Responsibilities:

Collaborate with the care team to manage and deliver patient care plans
Provide nursing support to patients in the Milford PA area
Meet critical deadlines for charting and documentation
Self-manage time and daily schedule efficiently
Utilize tablets mobile phones and EMR software effectively

Schedule:

Work schedule and shift expectations provided during onboarding

Travel:

Occasional local travel required

Qualifications:

Active Licensed Practical Nurse (LPN) license in Pennsylvania
Associate degree or higher
Minimum of one year of experience with electronic medical records (EMR) TruChart EHR or similar systems
AHA BLS or ACLS certification
Valid drivers license reliable transportation and auto insurance
Ability to travel locally as required
