Job Summary

Job Title: Physical Therapist Home Health
Location: Somerset PA 15501
Employment Type: Full-time

Overview: Provide in-home physical therapy services supporting patients in Somerset PA and surrounding areas. This role emphasizes team-based care collaboration and the delivery of high-quality patient care plans in a home health setting while supporting professional growth in a flexible and supportive environment.

Responsibilities:

Collaborate with the care team to manage and deliver patient care plans
Provide in-home physical therapy services to patients in Somerset and surrounding areas
Perform patient assessments and develop individualized treatment plans
Implement therapeutic interventions to improve mobility strength and functional independence
Educate patients and families on treatment plans exercises and safety
Maintain accurate and timely clinical documentation
Meet critical deadlines for charting and reporting
Self-manage daily schedule and time effectively
Utilize tablets mobile phones and EMR software

Schedule:

Monday through Friday 8:00am to 5:00pm
One weekend per month
Two holidays per year

Travel:

Local travel to patient homes
Approximately 20% of the day spent driving

Compensation and Benefits:

Estimated hourly rate of $55.00

Qualifications:

Bachelors or Masters degree in Physical Therapy from an accredited institution
Active Physical Therapist license in Pennsylvania
Minimum of one year of healthcare experience
Experience with electronic medical records (EMR) preferred
Valid drivers license
Ability to travel locally and spend time driving
Strong time management and organizational skills
