- Perform thorough physical examinations and health assessments to evaluate patient needs and conditions.
- Diagnose and manage acute and chronic illnesses developing treatment plans that include medication management and lifestyle recommendations.
- Collaborate with healthcare teams including physicians and specialists to ensure cohesive patient care and follow-up.
- Educate patients and their families on health management treatment options and preventive care strategies to promote wellness.
- Maintain accurate and timely records of patient encounters treatment plans and outcomes in electronic health records (EHR).
- Stay informed about the latest advancements in healthcare practices and contribute to quality improvement initiatives within the organization.
- Serve as a patient advocate providing support and guidance throughout the healthcare process while respecting patient autonomy and confidentiality.
Requirements
- Masters degree in Nursing (MSN) from an accredited program.
- Valid state NP license and current CPR certification.
- Previous experience as a Nurse Practitioner preferred; specialized training in areas such as family practice pediatrics or geriatrics is a plus.
- Strong clinical skills critical thinking abilities and excellent interpersonal communication skills.
- Demonstrated ability to provide compassionate patient-centered care while ensuring confidentiality.
