Job Summary

This position is contingent upon award:

The Primary Care Provider (PA/NP) delivers comprehensive evidence-based primary care for enrolled Veterans in a VA Community Based Outpatient Clinic. Practicing within the Patient Aligned Care Team (PACT) model the provider manages acute and chronic conditions preventive services and care coordination using in-person and virtual modalities. The role emphasizes quality safety access population health and Veteran-centered whole health care in alignment with VA directives and clinical standards.

  • Provide comprehensive adult primary care: assessment diagnosis treatment planning counseling and follow-up.
  • Manage chronic diseases (e.g. diabetes hypertension COPD CHF) preventive care and acute issues within scope.
  • Perform procedures consistent with privileges and training (e.g. EKG interpretation joint injections I&D suturing wound care).
  • Order/interpret diagnostics and therapeutics; coordinate referrals to specialty care and community partners.
  • Deliver care via in-person video and telephone visits; employ risk stratification and outreach to close care gaps.
  • Collaborate with RN care managers LPN/MAs behavioral health pharmacy social work and specialty services.
  • Lead/participate in daily huddles case reviews and panel management; ensure continuity and same-day access.
  • Implement clinical reminders standing orders and evidence-based pathways; ensure safe handoffs and closed-loop follow-up.
  • Practice in accordance with VA directives professional guidelines and state/federal regulations (HIPAA OSHA).
  • Contribute to quality improvement (PDSA cycles) peer review and performance measurement; participate in audits and chart reviews.
  • Report and help resolve safety events; follow infection prevention medication safety and test tracking policies.
  • Maintain panel access targets (third-next-available same-day access) and optimize scheduling/templates.
  • Reduce cycle time through efficient documentation in-rooming workflows and team-based task delegation.
  • Champion whole health trauma-informed culturally responsive care and service recovery when needed.
  • Document timely and accurately in the VA EHR (e.g. CPRS/Cerner); ensure appropriate coding and orders.
  • Use population health tools/registries to track chronic disease control preventive care and care gaps.
  • Protect Veteran privacy and information security; comply with documentation standards.
  • Completes required organizational compliance education including assigned requirements that are client-specified for Joint Commission Healthcare Staffing Services certification or other regulatory bodies.

Qualifications :

Required Skills and Knowledge:

  • Clinical excellence in adult primary care with sound diagnostic reasoning and escalation/consultation practices.
  • Team-based mindset with strong communication de-escalation and service recovery skills.
  • Data-driven approach to panel management access and quality; comfortable with dashboards and registries.
  • Competency in infection prevention medication management test tracking and emergency response in ambulatory settings.

Education Required:

  • Graduate of an accredited Physician Assistant program with current NCCPA certification OR Graduate of an accredited Nurse Practitioner program (MSN/DNP) with national certification (e.g. FNP or AGPCNP).
  • Current unrestricted license in the state where the CBOC is located; eligibility for VA credentialing and privileging.
  • Current BLS (AHA). ACLS may be required based on clinic scope; DEA registration/prescriptive authority per role and state law.

Nurse Practitioner:

  • At least 3 years of Adult Primary Care or Family Practice experience (residency/fellowship may substitute for some experience); new graduates with strong clinical rotations may be considered.


Physician Assistant:

  • At least 5 years of total approved academic training and health care experience.
  • Proficiency with EHRs and telehealth; ability to use clinical reminders and population health tools.
  • Experience Preferred:
  • Experience in VA/DoD CBOC FQHC or PCMH environments; familiarity with the PACT model and VA directives.
  • Womens health in primary care procedures (e.g. joint injections) or chronic disease program leadership.
  • Training/certification in quality improvement patient safety or informatics (e.g. Lean/Six Sigma CPHQ).
  • Bilingual skills and/or experience serving diverse Veteran populations.

Physical Requirements:

  • Sitting standing walking climbing bending/stooping kneeling twisting.
  • Visual acuity. Minimal hearing loss.
  • Push/pull/carry minimum 25 lbs.
  • Limited travel for training/meetings.

Other Special Qualifications:

  • DEA registration as applicable to scope and prescribing needs.
  • Obtain medical clearance and maintain current immunizations.
  • Eligibility to work in federal healthcare settings; successful background check/public trust suitability.
  • Completion of required trainings.

Additional Information :

Pay range is based on several factors and may vary in addition to a full range of medical financial and/or other benefits. Final salary and offer will be determined by the applicants background experience skills internal equity and alignment with geographical market data.
