WARM HEARTH AT HOME LOCATED IN CHRISTIANSBURG VIRGINIA IS SEEKING A HOSPICE CHAPLAIN

Position Summary

The Hospice Chaplain providesspiritual counseling and emotional support to hospice patients and theirfamilies. The Chaplain works as part of the interdisciplinary group (IDG) toensure patients spiritual needs are met while respecting their individualbeliefs and traditions.

Essential Duties andResponsibilities:

  • Provide spiritual care to patients and families as identified in the comprehensive assessment and individualized plan of care.
  • Participate in the hospice IDG meetings and contribute to the development implementation and review of patient care plans.
  • Document all visits assessments interventions and care provided in the patient record in accordance with agency policies and regulatory requirements.
  • Offer bereavement support services in collaboration with the Bereavement Coordinator.
  • Provide or arrange for appropriate spiritual services (including clergy of specific faiths when requested).
  • Be available for crisis intervention and urgent spiritual support as needed.
  • Respect patient and family values culture religion and confidentiality at all times.
  • Assist with funeral or memorial services when requested.
  • Participate in volunteer training and serve as a resource to staff and volunteers on spiritual issues.

Qualifications:

  • Ordination or commissioning by a recognized faith group required.
  • Master of Divinity or related degree preferred.
  • Experience in pastoral care preferably in hospice healthcare or related setting.
  • Ability to work as part of an interdisciplinary team.
  • Strong communication and counseling skills.
  • Must have reliable transportation and be able to travel to patient homes and facilities.
